01 / ARRIVAL WORKFLOW
Make the introductory appointment easy to prepare for.
A first appointment can involve registration, a studio orientation and a conversation with the instructor. Put the waiver task in the booking instructions alongside when to arrive and where to go. Tell the client what can be completed beforehand and what will happen in person.
If your studio offers several session types, use names clients recognize. A person booking an introductory equipment session should not have to choose between internal template names. Direct them to the intended form and let staff handle the underlying document structure.

02 / PEOPLE AND PERMISSIONS
Separate the signer from the package purchaser.
A package may be bought as a gift, or a parent may reserve sessions for a teenager. Confirm who will participate and which person must complete the signing process. Do not attach every participant in a package to the purchaser merely because that was the only email collected at checkout.
For a private session, allow a discreet review before the instructor begins. For a group class, define who handles a flagged response while the instructor prepares the room. The response needs an owner, not an assumption that somebody else saw it.
03 / BUILD THE FORM
Use form structure that supports a real handoff.
Start with identity, the relevant studio activity and reviewed participation wording. If a question is intended to prompt an instructor conversation, make that operational purpose clear. Avoid collecting a long history that nobody is assigned to review.
Keep consent and signature steps separate from general preferences. A guest should be able to understand which fields are required and what they are agreeing to. Preview the full form on a phone, including any lengthy text blocks, before publishing a new version.

Distinguish the participant from a gift or package purchaser.
Recurring bookings can create a misleading sense that every requirement is already handled. Give reception a consistent check: identify the client, locate the appropriate record and look at its current status. Keep attendance, package credits and waiver history as separate facts.
When a client moves from one program to another, follow the studio's approved procedure for the change. Do not silently alter an old signed document to describe the new activity. A newly published version and an earlier signed record should remain distinguishable in the history.

Rehearse the exceptions, not only the ideal booking.
Test a new client, a gift recipient and a teenager arriving with a different adult from the booking contact. Ask the desk to find each record without relying on the test organizer's memory. Then test a client who starts the form on a phone and finishes at the studio.
Write down what staff do when a record is missing or needs review. A clear escalation is more useful than an instruction to “sort it out before class.” Review the first week's points of confusion and improve the message or field responsible for each one.

Can the form replace the instructor introduction?
No. Use the form to collect the reviewed participation information and identify questions that need attention. Keep instruction, equipment orientation and individual conversations in the studio's normal process.
How do we handle package bookings?
Link each actual participant to their own required record. Track package credits and appointment attendance separately so a purchase does not become a substitute for signing.
