Give the new member a calm start before class.

The first class can already feel unfamiliar. A guest is looking for the room, checking their shoes, and trying to meet the instructor. Send the signing link with the class confirmation so the form can be read before that moment rather than squeezed into the introduction.

This outline connects the participant to a recognizable class or program, makes room for reviewed questions and participation wording, and finishes with a signature. It can support a trial-class workflow without pretending to be a complete membership or screening system.

What to decide before
you build the form.

Use the class name guests know.

Match the title to the booking message and instructor’s schedule. If different programs need different documents, keep the links easy to distinguish. A guest should not need to know your internal program codes to choose a form.

Keep screening separate.

Participation paperwork and exercise screening have different purposes. Ask the qualified team which questions belong in which workflow. A flagged answer can prompt review but should not act as a medical or fitness determination.

Plan for guest bookings.

Members often bring a friend or reserve a place under their own account. Collect the actual participant’s details and match them to the signed record. The person booking or paying is not necessarily the person joining the class.

Give this document a clear job.

This outline does not supply exercise clearance, membership billing terms, or a recommendation about training. Keep the instructor’s assessment and your approved screening process distinct from whether the signature has been received.

BEFORE THE FIRST SIGNATURE

Try the awkward cases.

Test a trial member, a guest booked by a friend, a youth participant with a guardian, and someone returning to a changed program. Review the phone reading order and ask the instructor how they will handle a missing form without holding up the rest of the class.

Once the wording and flow are reviewed, publish a controlled version. Keep the signed version with its record, and give the team a clear way to find a participant, resolve a missing signature, and retrieve the PDF later.

Can the link go in our class confirmation?

Yes. Place it beside the arrival instructions and explain who should complete it. A walk-in can use a QR code or kiosk to reach the same published form.

Is this a fitness screening questionnaire?

No. It is a participation-form outline. Your qualified team should select and manage the screening process separately.

Can we reuse it for personal training?

Review the actual service and document before doing so. The structure may be useful, but the approved wording and workflow need to fit the program being offered.
