A form for the person actually taking part.

An adult participation form works best when the signer can quickly recognize the activity, the business, and the request being made. Start with that context. A guest who booked through a friend should not have to guess whether the link is for their visit or somebody else’s reservation.

This outline puts participant details before the reviewed activity information, followed by any required acknowledgments and a signature. It gives you a useful reading order without supplying a generic release clause. The wording still needs to match your operation and the people taking part.

What to decide before
you build the form.

Name the activity clearly.

Use the name guests see in their booking or first-visit instructions. If several activities need different documents, keep their template titles distinct. That helps both the guest selecting a form and the teammate retrieving a record later.

Choose details you can use.

Ask for the participant’s name and the contact information your workflow needs. A booking contact and a participant can be different people. Decide how the two records will be matched before adding more questions to the form.

Give flagged answers an owner.

A question can route an answer to staff attention, but someone must know what to do next. Add questions only when your team has a defined review process. Keep acknowledgments in the positions agreed during document review.

Give this document a clear job.

A participation signature does not settle every requirement around a visit. Payment, equipment checks, orientation, and activity approval may each need a separate step. Do not turn the final signature into a shortcut for those decisions.

BEFORE THE FIRST SIGNATURE

Try the awkward cases.

Try an adult who booked for themselves, a guest booked under another person’s email, and a returning guest whose old form used different wording. Check the phone experience and confirm that staff can locate the participant and the exact published version they signed.

Once the wording and flow are reviewed, publish a controlled version. Keep the signed version with its record, and give the team a clear way to find a participant, resolve a missing signature, and retrieve the PDF later.

A few useful answers.

Is this a completed liability release?

No. It is a form outline with a practical field order. Add the document reviewed for your activity and location before using it with participants. A familiar title or signature field does not establish that the wording is suitable.

Should the booking contact sign for everyone?

Do not assume so. Use your approved signer requirements and identify the actual participant. The booking contact can share a link without becoming the signer for the rest of a group.

Can I add initials or questions?

Yes. The builder supports initials, questions, and other blocks. Add them where the reviewed document and staff process call for them, then test the resulting reading order on a phone.
