A zipline participation outline needs a clear activity name before it needs more fields. Guests may buy a tour by itself or as part of a package. Tell them what the reviewed document covers and which participant the form is for.
Keep the form readable ahead of the timed arrival. The document check supports reception, while fitting, instruction and course decisions remain with the operating team. This separation makes the meaning of a completed signature more precise.
