01 / ARRIVAL WORKFLOW
Treat trials, classes and parties as different arrival patterns.
A recurring class usually has a known roster. A trial session includes a family learning where to go. A party can bring many children at once, with a booking parent who does not know which guardians have signed. Build the instructions around the visit type instead of sending one vague reminder.
For parties, give the organizer a shareable participant message early. Ask the actual guardians to complete the form, and review missing signatures before the group reaches reception. Keep a place for on-site signing away from the main arrival line.

02 / PEOPLE AND PERMISSIONS
Reach the adult responsible for the form.
A child may arrive with another family or a carpool. The accompanying adult is not automatically the signer under your center's requirements. Explain the guardian process before the visit and give staff a clear escalation when the required signature is missing.
List each participating child rather than attaching a family-level label to an unnamed group. Siblings can attend different classes and parties, so reception needs to identify the child associated with the relevant record. Keep pickup and class-placement decisions in their own operational procedures.
03 / BUILD THE FORM
Keep the form readable for a busy household.
Use a clear order: guardian identity, participating children, activity context, reviewed wording and required signing steps. Avoid repeating the same introductory text above every field. Long blocks of approved text still need readable spacing and headings on a phone.
Questions that trigger staff review should have a named reviewer and an agreed next step. Do not ask coaches to discover a flagged answer by chance after class starts. Test how the handoff works while another staff member is greeting the next family.
Make first visits and returning visits distinguishable.
A returning student might have an existing valid document, while a visiting sibling has never been entered. Check the actual child and record rather than recognizing the surname. The class register helps identify attendance, but it does not replace the signed document history.
When you publish revised wording, preserve the earlier version with existing signatures. Give staff a short explanation of who needs the new form under the center's policy. Avoid a blanket instruction to collect another signature whenever anything about the booking changes.

05 / PREPARE YOUR TEAM
Rehearse a party arrival before using it live.
Use a small sample roster with completed, missing and needs-review records. Have one person act as the organizer and another as reception. Check whether staff can locate the right child without asking the organizer to remember every guardian's email address.
Finish the test by recording the arrival and moving the participant into your normal activity preparation. Signing status does not replace supervision, instruction or other center requirements. Put unresolved questions with a designated lead so the reception team is not improvising at the busiest moment.
